Dr. Conrad Ballecer, Jr., 2000 TOFA, of Phoenix, Arizona wrote in
his letter of recommendation: "Herewith is the application form for
our nominee for the TOFA 2002 including his CV.  His name is
Luis Tan, MD, a long time resident of Phoenix, Arizona since 1957
to the present.  He is of Filipino-Chinese lineage but quite a
Filipino by actions, emotions and persuasions. He is a
Pediatrician and Allergologist.  He is quite well-known far and
wide among the local Filipino community here in Phoenix.  He
was the founding Godfather and First President of the Filpino Club
of Arizona and had been president for the first 10 years of its
existence. Almost single-handedly and with the use of his own
financial resources, shaped and nurtured quite lovingly,
generously and protectively to what is now a very well-respected
ethnic club and in the process produced and trained a great
number of the present leaders of the different social, civic and
professional organizations in the local community. His
thoughtfulness and generosity know no bounds; from welcoming
incoming new Doctors in the City of Phoenix; bailing out Nurses
from prison from his own funds thus saving these Nurses from
certain deportation; to taking care of the sick elderly Filipinos
without recompense, etc., etc., etc. I respectfully and with the
greatest pride and honor nominate this outstanding Filipino,
Dr. Luis Tan for the coveted honor and recognition as one of the Twenty Outstanding Filipino American for
2002."

Dr. Luis Tan was born in Wright, Samar.  He gruaduated at Samar High School in 1949 and proceeded to the
University of Santo Tomas for his pre-medicine studies. He attended medical school from 1951-1956 also at
UST, Faculty  of  Medicine & Surgery. He came to the U.S. in 1956 in Milwaukee, Wisconsin to specialize in the
field of Pediatrics.

Dr. Tan started private practice in Pediatrics & Allergy in Phoenix, Arizona in 1962 up to the present.  He is a
Diplomate in the American Board of Pediatrics and a Fellow for the American College of Allergy and
Immunology, the American Academy of Pediatrics and the Westcoast Allergy and Immunology.

For his community affairs involvement, he founded and became the first President of the FilipinoClub of
Arizona in 1966, and was instrumental in the acquisition of the only Philippine Community Center in Phoenix,
Arizona. He is widely and famously known among Filipino practicing physicians and was awarded lifetime
achievement meritorious distinguished service from the Filipino hierarchy.  Dr. Luis Tan was given the
Outstanding Citizen Award in 1994 by the Arizona Asian-American Association for his significant contribution
to the advancement of the Asian community. His representation of Filipinos in Arizona will be a pride and joy
of all that will be cherished with dignity and emulation.

In the field of Medicine, Dr. Tan's residence  was in the Maricopa General Hospital in Phoenix, Arizona from
1957 to 1958. In Pediatrics, it was also  at the same hospital wherein he was a Chief Resident from 1958 to
1960, and his fellowship was at the Allergy & Immunology National Asthma Center in Denver, Colorado. He is
affiliated with eight hospitals in Phoenix.

Dr. Luis Tan's professional affiliations would include the American Medical Association; Arizona Medical
Association; Maricopa County Medical Society; Arizona Allergy Society; Greater Phoenix Allergy Society; Los
Angeles Pediatric Society; Association of Philippine Practicing Physicians of Arizona; Member of Board of
Directors, AZ Medical Network; Healthcare Consortium of AZ. He was the Chairman of the Scientific
Committee of AZ Medical Association. He is a member as well of the AZ Medical Assocation, Committee on
Maternal and Child Care;Grievance Comittee,Maricopa County Medical Society; Credentials, Quality
Assurance & Utilization Committees of Health Choice, Phoenix Memorial Hospital, St. Joseph's Hospital;
Infection Control Committee of Phoenix Children's Hospital, Good Samaritan Hospital, Phoenix Baptist
Hospital, St. Joseph's Hospital; Pediatric Committee of J.C. Lincoln Hospital, Phoenix Baptist Hospital,
Maryvale Hospital, Memorial Hospital, Paradise Valley Hospital, Pharmacy & Therapeutic Committee of
PacificCare Security Horizon, Health Choice, Memorial Hospital; Medical Records Committee of Phoenix
Children's Hospital, Good Samaritan Hospital, Phoenix Baptist Hospital and Memorial Hospital. He was
elected to a 3-year term from January 1998 to December 2000 as the Board of Directors of the Arizona
Foundation for Medical Care.

Dr. Tan was the founding President  and president for the first 10 years of the Filipino Club of Arizona;
Commissioner, Pacific Rim Advisory Council, City of Phoenix from 1995 to the present; Member, 1995
Phoenix Census AD HOC TASK FORCE and received an award for outstanding service and dedication. He
was also a Volunteer at the St. Vincent De Paul Medical Clinic since 1995 to present and is still volunteering
every 3rd Wednesday of each month.

Sharing his numerous blessings and achievements are his loving wife, Mary Josephine Flynn and his three
children, Rosemarie (also an MD), Michelle and Geoffrey. #
